At Biltmore Dermatology, we solved the access problem by establishing Indiana’s very first Walk In Dermatology office in 2021. You don’t have to wait weeks or months for an appointment to access high-quality medical and cosmetic dermatology care anymore. Patients may schedule an appointment or simply Walk In Now. Biltmore Dermatology’s innovative “Walk In Now” Dermatology office model modernizes and solves the access to dermatology care problem. We not only treat patients’ medical skin care needs by providing full body skin examinations, diagnose and treat everything from acne, rashes, and skin cancer right in our state-of-the-art facility but also extend this ultimate convenience to our cosmetic dermatology services.
FASTER IS BETTER
There are 5.4 million new cases of skin cancers diagnosed every year in the United States. The faster you diagnose skin cancer the more treatable it is. We offer Image-Guided Superficial Radiotherapy (IG-SRT) as a First-Line Non-Invasive Treatment for non-melanoma skin cancer with a 99.3% cure rate. No cutting. No pain. No scarring. No downtime. Also, the faster you can be seen to have acne, eczema, psoriasis, other rashes, or many other skin conditions diagnosed and treated the less chance you have of experiencing scarring, spreading, or worsening of your symptoms.

APPOINTMENTS
We strive to give all patients the earliest available appointment. We request 24-hour notice for cancellations, as this will enable us to fill the appointment with another patient. Without proper notice, there is a $50 fee for missed general dermatology appointments, and a $150 fee for any missed surgical, laser, or cosmetic appointments.
We also reserve the right to dismiss you from the practice for repeated missed appointments or if you are sent to collections for non-payment of services after 90 days.
Biltmore Dermatology uses an automated phone/text system for reminders of appointments. You understand this is a courtesy and Biltmore Dermatology is not responsible for missed reminder calls/texts. You are responsible for all appointments made.
WHAT TO BRING
- A photo ID
- Your most recent insurance card(s), and any Medicare supplemental insurance card(s).
- A complete list of the medication (prescription and non-prescription) you are currently taking.
- A referral from your primary care physician, if necessary/required by your insurance plan.
PAYMENT FOR SERVICES
You are responsible for paying your annual deductible, co-payments, and charges for any non-covered cosmetic services, or medical-grade products at the time of service. We accept cash, check and Amex/Discover/MasterCard/Visa. A service charge of 4% will be applied to all credit card purchases/payments in Square. We will bill your insurance company, and we do accept assignments from them. Any remaining balance will be billed to you. If you do not have insurance, or do not have your insurance card with you for your visit, full payment will be necessary at the time of service. We utilize a third-party billing company who handles all claims and answering your billing questions. Our third-party billing company utilizes paperless billing. All billing statements will be distributed to you via text message or email. This ensures patients are informed in the quickest possible way and helps improve their carbon footprint. SQUARE TERMS AND CONDITIONS POLICY
Biltmore Dermatology utilizes Square for processing payments in our office which include but are not limited to: bill payments, copays, deductibles, cash and carry prescription medications, cosmetic services, medical grade products, and other services.
EMV-enabled chip cards: the card will be tapped, dipped, or inserted – so the chip can be read and provide and additional layer of protection against fraud for all in person purchases made in the office. Your signature gives your consent to your credit card being used in Square to process these payments or purchases.
PROOF OF SERVICES RENDERED
All transactions are audio and video recorded to document proof of purchase and receipt of services defined above. In the event of any dispute these recordings will be sent to Square and your financial institution. You will receive an itemized receipt which outlines any surcharge fee and products/services purchased.
All prescription and medical-grade product sales are final.
PRESCRIPTION REFILLS
Please call our office during regular business hours to request prescription refills. We try to accommodate these requests on the same day, though it is not always possible. Return calls will only occur if there is a problem or question regarding your request; you should call your pharmacy to see if the refill has been called in. Yearly visits are necessary to maintain a status as an active patient and to qualify for prescription refills.
TELEPHONE CALLS
We understand how frustrating it is to be stuck in a phone messaging system when you call an office. Below you will find a few extensions that you may dial as soon as the message system is engaged. If you leave a message on the scheduling line, you will receive a call back within 24 hours.
Our phones are on from 8:00 am to 6:00 pm Monday and Wednesday, and 8:00 am to 5:00 pm Tuesday and Thursday. Our office is closed from 12:00 pm to 1:00 pm daily for lunch. Friday we are open from 8:00 am to 12:00 pm for Image Guided Superficial Radiation Therapy (IG-SRT) patients only.

If you have an urgent medical need outside of office hours, please go to the nearest emergency room. Routine prescription refills and lab/pathology results are not available after business hours. FINANCIAL RESPONSIBILITY STATEMENT
If no insurance is to be filed by Biltmore Dermatology, full payment is due at time of service. Should valid insurance be provided to the office after the date of service and denied for timely filing, patient will be responsible for the full balance due. Co-payments, co-insurance, deductibles and non-covered services are due at time of service as well. A $150.00 fee will be assessed for missed surgical appointments without 24-hour notice. A $50.00 fee will be assessed for all other missed appointments without 24-hour notice and for returned checks. A $30 fee will be assessed for copying your medical records.
UNPAID ACCOUNTS
Any payments, which are due, including those starting 30 days after insurance coverage has been completed, you will be charged a $25 monthly service charge or a rate of 10% interest per month based on the unpaid balance, whichever is larger. You understand that you are financially liable in the event of nonpayment; you agree to pay court cost(s) and reasonable attorney fees. Accounts not cleared within 90 days will be sent to a collection agency and may cause an adverse incident on your credit report. You understand and agree that if a check is returned for insufficient funds, Biltmore Dermatology will only accept cash or credit card payment(s) thereafter and you will be obligated to pay a returned check fee of $50. Balances must be settled. If your account is sent to collections for an unpaid balance, you may be discharged from the practice, and not eligible to return.
AUTHORIZATION TO PAY BENEFITS TO BILTMORE DERMATOLOGY AND RELEASE INFORMATION
I hereby authorize payment directly to Biltmore Dermatology, realizing I am responsible to pay non-covered services. I also authorize Biltmore Dermatology to release any information acquired in the course of my treatment necessary to process insurance claims.
IG-SRT, Lab tests, Pathology charges, Precertification, and Procedures:
Please note that all procedures have additional costs, which may not be included in regular office visit fees. These procedures include but are not limited to IG-SRT (Image Guided-Superficial Radiation Therapy), PDT (photodynamic therapy), cryotherapy (liquid nitrogen treatment) to burn off lesions, acne procedures, chemical peels, biopsies, surgeries, injections, mole removals, and wart treatments. If your visit includes biopsies, lab tests, or cultures, the specimens are sent out to an outside lab for processing. You will receive separate billings from the laboratory/facility performing the service. You are responsible to notify us if your insurance company requires particular labs for coverage of the processing. If your insurance requires pre-certification, it is your responsibility to make sure it is obtained. You will be held financially responsible if the pre-certification is not obtained.
CONSENT FOR MEDICAL TREATMENT OF A MINOR:
I (we) the undersigned parent, parents, or legal guardian of a minor do hereby authorize and consent to any medical exam or treatment rendered under the general or special supervision of a duly licensed provider, licensed under the provisions of the laws in the State of Indiana. It is understood that this authorization is given in advance of any specific diagnosis, treatment or medical care being required but is given to provide authority and power to render care, which the aforementioned provider in the exercise of his/her best judgment may deem advisable.
It is understood that effort shall be made to contact the undersigned prior to rendering treatment to this patient, but that any of the above treatment will not be withheld if the undersigned cannot be reached.
CONSENT TO TREAT:
I request and give consent to my provider to provide and perform such medical/surgical care, tests, procedures, drugs, and other services and supplies as are considered necessary or beneficial by my provider for my health, and well-being. I acknowledge that no representations, warranties, or guarantees as to the results or cures have been made to me or relied upon by me.
RELEASE OF PROTECTED HEALTH CARE INFORMATION VIA TELEPHONE TO ANSWERING MACHINE OR VOICEMAIL:
I give consent and authorization for the medical staff of Biltmore Dermatology or the third-party billing company to leave Protected Health Information about me or for me on my answering machine or voicemail via the telephone number I have listed. I understand I may revoke this privilege at any time by submitting my request in writing to this office.

If you have an immediate concern, you want to find quality care as soon as possible. Our office takes walk-in appointments and will be happy to address your concerns and offer you immediate treatment suggestions and medical care.

Ryan is State Licensed and Nationally Board Certified as a Family Nurse Practitioner (NP-C) by the American Academy of Nurse Practitioners. Additionally, he is board certified as a Dermatology Certified Nurse Practitioner (DCNP) by the Dermatology Nursing Certification Board. He has 20+ years of Dermatology Experience.
